ENROLLMENT FORM

BIOGRAPHY:

Dr. Lauren Schack Clark has been teaching piano to pre-college and college students for over four decades.  She is Professor of Piano and Keyboard Area Coordinator at Arkansas State University.  She has performed throughout the US, Europe, and China.  A complete biography may be found on her website.

TUITION:
Tuition is due on the first of each month.  It is based on a series of experiences and involvement that will provide your child with optimal learning outcomes.  Items covered by Tuition include:
  • Weekly lessons
  • Periodic Studio Classes
  • Recitals: programs, refreshments
  • Preparation for performances, festivals, and competitions outside lesson time
  • Lesson planning, development of curriculum, scheduling, bookkeeping
  • Office supplies
  • Teacher professional development

Tuition amounts:  Please email [email protected] for more information.

LATE PAYMENT:
If payment is not received by the 5th of the month, a late fee of $10 will be assessed the following month.

LESSON CANCELLATION:
Twenty-four hours notice is required for all cancellations.  Unexcused absences will not be made up, credited, or refunded.  Lessons missed by the teacher will be made up.

TARDINESS:
If a student is late for their lesson, the lesson will end promptly at the end of the scheduled time frame.

ILLNESS:
Students and family members who are ill should not come to piano lessons. The teacher reserves the right to send a student home if they arrive sick.

INCLEMENT WEATHER/CAMPUS CLOSINGS:
If Arkansas State University is closed due to inclement weather, we will not have lessons, and all efforts will be made to make these lessons up.

MUSIC/MATERIALS PURCHASES:
Parents/Guardians will be sent links to music to be purchased.  Please buy music promptly so your child can practice and enjoy their new music as soon as possible.

PRACTICE EXPECTATIONS:
  • Students are responsible for practicing their full assignment and preparing the lesson that has been assigned to them.
  • Parents/Guardians and the teacher will discuss an individual plan for the amount of time spent practicing each day.
  • Students should practice at least 5 days a week.
  • Give practice the same priority as homework.
  • Schedule practice at a regular time(s) each day.
  • Practice may be broken up into smaller sessions to help maintain concentration.
  • Avoid distractions such as phones, pets, television, visitors, or other interruptions.
  • Regularly review items in the shared online Practice Log, which should serve as a guide to daily and weekly goals and progress.

PARENTAL/GUARDIAN INVOLVEMENT:
  • The adult’s most important role is to listen to the student with encouragement and enthusiasm.  It is recommended that the parent or guardian sit down the the student at lease once a week to offer support and see that all assignments are being practiced.
  • Parents/Guardians may sit in on lessons, especially with younger children. The exception is when younger siblings are present, or when it would cause distraction to the student.  This should be discussed with the teacher.  They should take notes on what is to be practiced, so they can assist with and guide practice at home.  They do not need to have a musical background themselves.

FESTIVALS AND RECITALS:  Students are expected to participate in as many of these below as possible.
  • J. D. Kelly Competition (Fall)
  • Sunday Sampler Recitals (Fall and Spring)
  • National Federation of Music Clubs (NFMC) Festival (February or March)
  • Arkansas State Music Teachers Association (ASMTA) Region II Festival (April); Winners go on to State in June.
  • Piano Guild (May)

LOCATION, PARKING:
Lessons are held at the A-State Fine Arts Center, 2412 Quapaw, Room 243.  Park in Visitors spaces near building.  See Parking Map.

DROP OFF, PICK UP:
Please be on time for lessons and other activities, and pick up children promtply after lessons.

CALENDAR:
We will follow the A-State Calendar for Fall and Spring holidays.

INSTRUMENTS:
Students must have daily access to an instrument in good working order.  Digital pianos are acceptable, but acoustic pianos are preferred.  Amro Music in Memphis has a wide selection of instruments available for sale or rental.  Consult with the teacher before purchasing an instrument.  Acoustic pianos should be tuned at least once a year.

METRONOME:
Students should have access to a metronome or metronome app whenever they are practicing.  Pro Metronome is a very good free app.

SUMMER LESSONS:
Students may take summer lessons for an hourly rate.  Parents and the teacher will decide on the number of lessons per month and parents will be billed accordingly at the beginning of the month.

SIGNATURE:
I have read the Clark Piano Studio Policy for 2026-2027 in its entirety, and I understand and agree to its conditions.
